 NR436 Community, Public, and Population Health Nursing

DIRECTIONS

4. Epidemiological Data: In one or more paragraphs, compare the county, state, national, and world prevalence and mortality data. Discuss how data are similar and how they are different. Explain if the community health concern is improving or worsening. Provide a citation for at least one scholarly source.
5. Community and Environmental Assessment Data: In one or more paragraphs, discuss the findings and trends in your community and environment assessment and how they relate to your practicum topic. Examine findings for evidence of the social determinants of health (SDOH). Identify two strengths and two areas for improvement within the community. Provide a citation for at least one scholarly source.
6. Educational Material: In one or more paragraphs, discuss your educational material, how this material is appropriate for the community, and how it will help licensed healthcare professionals and community members improve health. Provide a citation for at least one scholarly source.
